How do I apply to become a supplemental educational services (SES) provider under the No Child Left Behind Act?

U.S. DEPARTMENT OF EDUCATION:

Individuals or groups of individuals interested in becoming supplemental education service providers must organize as a legally recognized non-profit or for-profit entity and be approved by their state. Each state develops a provider application and approves the providers for their state.
